- मैं मुख्यमंत्री ममतान योजना के माध्यम से सेवा प्राप्त करने के मिए अपनी सहममत देता|दिती हूँ ।
- मैं पुमि करता/करती हूँ मक मुझे सेवा के मिए आवश्यक सभी जानकारी और दस्तावेजों के बारे में समझाया गया है । मैं सभी पात्रता, आवेदन जमा करने की प्रमिया, शुल्क, समयसीमा और सेवा से संबंमित अन्य चीजों के बारे में पूरी तरह सेअवगत हूँ।
- मुख्यमंत्री ममतान योजना के माध्यम से सेवाओं का छिभ उठाने के मिए, मैंने शुल्क (मवभागीय शुल्क (यमद कोई हो) + ममतान सेवा शुल्क) के साथ सभी आवश्यक दस्तावेज ममतान फील्ड एग्जीक्यूमिव को स पं मदए हैं ।
- मैं ममतान फील्ड एग्जीक्यूमिव को दस्तावेजों की तस्वीर छिने/मिर्माणि प्रमतमिमप बनाने के मिए प्रामिकृत करता/करतीहूँ|मजसकाउपयोगकेविमुख्यमंत्रीममतानयोजनाके मिएमकयाजाएगा।
- उपयुक्त सेवा के मिए सरकारी मवभाग द्वारा अमतररक्त दस्तावेज मक मांग करने पर मैं दस्तावेज ममतान फील्ड एग्जीक्यूमिव को प्रस्तुत करं गा/ करूँ गी |
- यमद मेरे आवेदन को , मेरी तरफ से मकसी चूक / या मकसी अन्य वास्तमवक कारण से मवभाग द्वारा अस्वीकार कर मदया जाता है , तब हस स्थथमत में , मैं न तो ममतान फील्ड एग्जीक्यूमिव को मजम्मेदार ठहराउँ गा/ठहराउँ. गी और नही आवेदनजमाकरतेसमयदीगईटामशक्तिगनेकादावाकरं गा/करं गी।
- मैं सहममत देता/ देती हूँ मक, ममतान फील्ड एग्जीक्यूमिव के वि प्रमाण पत्र की सुपुदुगी के समय मेरे साथ फिशी खीचं सकते हैं। सम्बंगित मवभाग मुख्यमंत्री ममतान योजना कायिम के प्रचार/मवज़ापन के उद्देश्य से उस तस्वीर का उपयोग कर सकता है।
- मैं उपयुक्त सभी मनयमों और शर्तों से पूरी तरह से अवगत और सहमत हूँ ।
- मैं घोषणा करता/करती हूँ मक मेरे द्वारा स्वेच्छा से प्रस्तुत की गई सभी जानकारी सत्य, सही और पूणु है।
Citizen’s consent to avail service through Mukhyamantri Mitaan Yojana
- I hereby give my consent to avail service through Mukhyamantri Mitaan Yojana
- confirm that have been explained about all information and documents required for service. am fully aware about all eligibility, procedure of application submission, applicable fees timeline and other things related to the service.
- As a part of availing services through Mukhyamantri Mitaan Yojana, handed over all the required documents along with the applicable fees (Departmental fees (if any) + Mitaan Service charges) to the Mitaan Field Executive.
- do authorize Mitaan Field Executive to take picture/ make digital copy of the documents, which shall be used exclusively for the purpose as stated above.
- In case of any additional document/s required by the government department as a part of the above service, shall submit the same to the Mitaan Field Executive.
- I shall neither make the Mitaan Field Executive held responsible nor claim to return the amount given at the time of submitting the application, in case my application gets rejected by the department due to lapses at my end and/or for any other genuine reason.
